Entry & progression
Entry: you must already hold — or qualify for — the appropriate national engineer endorsement. Progression: the STCW endorsement is a second layer printed on the same credential. Read this first. Everything else in this course depends on getting the stacking right.
A US merchant mariner credential can carry two kinds of engineering endorsement. The national endorsement is American law — Chief Engineer, Third Assistant Engineer, Designated Duty Engineer, and ten others, all listed at 46 CFR 11.501. The STCW endorsement is the international convention — OICEW, Second Engineer Officer, Chief Engineer Officer, ETO. They are printed on the same document. They are not the same thing.
The rule that governs the stack
WARNING — 46 CFR 11.201(a) — the national endorsement comes first "An applicant for any STCW endorsement must hold the appropriate national endorsement unless otherwise specified."
Read those last three words. They are not decoration. There is exactly one place in the engineering regulations where something is otherwise specified — and it is the ETO. Module 4 is about nothing else.
So the ordinary shape is: earn the national ticket, then bolt the STCW endorsement on top of it. You do not get an STCW engineering endorsement by walking in off the street with a pile of sea time and a course certificate. The Coast Guard wants to see the American licence underneath it.
The asymmetry
The two layers are not symmetrical, and the asymmetry is worth memorising because it decides which vessels you can sail on.

National without STCW — 46 CFR 11.323(c), verbatim
"An engineer officer who does not hold an STCW endorsement may serve on seagoing vessels propelled by machinery of less than 750 kW/1,000 HP, the vessels specified in § 15.105(f) and (g), and vessels operating on the Great Lakes or inland waters of the United States."
Three doors, then. Small machinery. The 15.105(f) and (g) vessels — small vessels on domestic near-coastal voyages, which the Coast Guard deems compliant with the convention. And the Great Lakes and inland waters, which the convention was never written for. A DDE running a 600 kW harbour boat needs no STCW endorsement at all, and never will.
What this means for a yacht engineer
It means the question is never "do I need STCW?" in the abstract. It is "does this vessel, on this voyage, trigger it?" A 40-metre motor yacht doing charters out of Fort Lauderdale and never crossing the boundary line is a different regulatory animal from the same yacht crossing to the Bahamas. Lesson 1.4 tells you exactly where the line is.
NOTE — One credential, two layers, two rulebooks The national layer is scoped by horsepower and gross tonnage. The STCW layer is scoped purely in kilowatts. They are measuring different things with different rulers, and they cap each other independently. You can hold an unlimited-tonnage national ticket and a restricted STCW ticket at the same time — and plenty of engineers do.
